What to bring to your first appointment

You’ll need your doctor’s name, your medical history, including medications, and any allergies you may have. Please also bring your health insurance information and booklet if you have them. All of this information is required when you fill out the new patient forms.

Financial policy

Payment is due at the time of treatment. We offer payment plans to assist in covering the costs of your treatment. We accept assignment from insurance companies. Ask our front desk staff for more details.

Insurance

Patients are responsible for the full cost of treatment, including any costs not covered by insurance. We will work with you to help you maximize your insurance coverage for your treatment and we can submit your claim directly to your insurer.
